Q2. 如何異動眷屬健保？

ANS:

(一)眷屬加保：

請至本室下載申請表加保並附戶口名簿影本、健保轉出證明、子女年滿廿歲以上附學生證影本(應屆畢業生自當學年度終了之日起一年內無職業或服兵役退伍自退伍日起一年內無職業者，須附畢業證書或退伍令及身分證影本等；若為國外回國者，有國內戶籍可立即加保，無戶籍資料者須辦妥遷入後，滿四個月才可加保，加保時須檢附戶籍謄本影本。

(二)眷屬退保(轉出)：

請至本室下載申請表退保(服役者須附兵役通知影本)，子女年滿廿歲以上未在學者，先辦轉出後再到戶籍所在地辦理轉入加保。

Q1. 教職員工如因留職停薪或出國，如何辦理健保停保/續保/復保作業？

ANS: 員工本人停保、復保異動說明如下。

如眷屬偕同出國，一併辦理停保；眷屬未出國則申請改按其他身份投保。詳細說明依健康保險局公告辦理。

停保

1. 如員工留職停薪未出國者，可將健保轉出至戶籍所在地之鄉鎮市區公所辦理續保
2. 如員工預計出國大於六個月者，請依中央健康保險局〈停保〉規定辦理

續保

1. 一般留職停薪：被保險人應自付全額(自付+學校負擔)保費，留職停薪生效日前先將保費
繳交至出納組。
2. 育嬰留職停薪：被保險人可選擇：(1)按月繳納─健保局另寄繳款單給被保險人，持繳款
單按月自行繳交；(2)遞延三年繳納。

復保

停保後回國要辦辦理復保，應備齊護照(有姓名、身份證號)、入出境紀錄表及身份證影本，送人事室辦理復保作業。

說明：辦理停保、退保時，持有之健保IC卡應自行保管，無須繳回且不得再使用，但於重新復保或加保時，原持有之健保IC卡辦理「更新」後即可繼續使用。
